چکیده
A 40 year old woman with rheumatoid arthritis developed pure red cell aplasia after treatment with D-penicillamine 500 mg/day. D-Penicillamine was stopped and short term treatment with steroids resulted in complete recovery of bone marrow.

Fatal aplastic anaemia and liver toxicity caused by D-penicillamine treatment of rheumatoid arthritis.
A 65 year old woman with rheumatoid arthritis developed marrow aplasia and jaundice owing to D-penicillamine treatment. Recovery of bone marrow was ineffective, and the patient finally died despite intensive therapeutic measures. The rare coexistence of myelotoxicity and hepatotoxicity is presented and discussed.
متن کاملAcquired sideroblastic anaemia after aplastic anaemia caused by D-penicillamine therapy for rheumatoid arthritis.
A 68 year old man with rheumatoid arthritis developed marrow aplasia during D-penicillamine treatment. Recovery of granulopoiesis and erythropoiesis was ineffective with features of a secondary sideroblastic anaemia. Absence of megakaryopoiesis persisted. Therapeutic measures failed, and the patient finally died. These events illustrate a haematopoietic stem cell injury induced by D-penicillamine.
متن کاملRed cell aplasia following prolonged D-penicillamine therapy.
Red cell aplasia developed in a case of Wilson's disease following an increase in D-penicillamine dosage after 14 years' treatment. In vitro study of the effect of D-penicillamine on 59Fe incorporation by marrow cells did not suggest that the patient's erythropoiesis was particularly sensitive to D-penicillamine or determine the mechanism of drug toxicity. However, three weeks after the drug wa...
متن کاملPyoderma gangrenosum with pure red cell aplasia.
Pyoderma gangrenosum is an inflammatory condition of the skin commonly associated with inflammatory bowel disease and rheumatoid arthritis, but also associated with various hematological malignancies. We describe its association with pure red cell aplasia in a four year old boy who presented with progressive skin lesions, fever and pallor, and improved with corticosteroid therapy.
متن کاملImmunosuppressive mechanisms in pure red cell aplasia--a review.
Pure red cell aplasia (PRCA) has been associated with a variety of clinical disorders, and various autoimmune mechanisms have been described to account for the red cell suppression. Primary PRCA occurs via both humoral and cell mediated mechanisms. Recent evidence using gene rearrangement studies indicates PRCA with T-lymphocytosis is a clonal chronic T cell lymphoproliferative disorder in whic...
